Before you start filling out forms, there are a number of steps you must first complete to fulfill the requirements of Boston Public Schools.
Social Security Card and ID
First, you'll need your Social Security card. If you don't have it, you can get a new one mailed to you by clicking below - it will take 5-7 business days to be mailed to you.
You will also need another form of government isued photo ID, such as a driver's license or passport. It cannot be expired.
Fingerprinting
Next, you can schedule an appointment to have your fingerprints taken. They can often be scheduled within 24 hours and take 5-10 minutes to complete.
Once you've had your fingerprints taken, you'll need the receipt that you receive for the next step.
When scheduling, use this BPS Provider ID: 00350000
Official Transcripts
You will need a PDF copy of your official transcripts from your undergraduate degree.
If you don't currently have a digital/scanned copy of your official transcripts, you should request them now from your alma mater.
Ethics Training
Boston Public Schools requires all teachers to go through Conflict of Interest Law/Ethics Training Certification.
Once you are done, take a screenshot of the certificate to upload later.
These forms must be filled out on a laptop or desktop, as the form fields do not appear on mobile devices.
Step 1 is a W-9 form, which will allow us to match you with opportunities outside of Boston Public Schools (BPS) as a contractor.
All forms that are required by BPS have been collected into one PDF document in Step 2. You will be a direct employee of BPS as a substitute teacher.
Once you fill in all required fields and submit, your information will be saved in a document on PDF filler on a secure server. Learn more about the security of your information here.
Please read through all of the forms carefully, as your digital signature is legally binding.
Please note that you will not be able to save a partially completed document to come back to later.
Once you've submitted your completed PDF, you're all done!
